We are a skilled nursing and rehabilitation center, providing inpatient short-term rehab and/or long-term care to elderly or disabled individuals.

Maintaining The Maximum Health

Licensed and certified skilled nursing facility with Medicare and Medicaid. We provide either short-term rehabilitation or long-term care on an inpatient bases 24/7 for someone recovering from an acute condition (i.e. stroke or fractured hip) or an elderly and/or disabled individual who needs care for a chronic condition (i.e. complications from diabetes or heart disease).

The rehabilitation program consists of physical therapy, occupational therapy and speech/language therapy.

We are the only skilled nursing facility in western Palm Beach County, Florida; serving Belle Glade, Pahokee and South Bay. We setup transportation to dialysis care and other outside medical appointments as necessary. We have primary physicians who make regular rounds to monitor the patients and residents. The staff have a lot of longevity and experience working together as a team.

We are a reputable skilled nursing facility that has been rated 4-5 stars by the state AHCA. Certified by Medicare and Medicaid and licensed by the State of Florida. Also, received a Joint Commission accreditation in 2021. We are the only nursing home provider and the nursing staff “Employer of Choice” in this area of Western Palm Beach County.

Our facility invites family and friends to visit 7 days a week and vaccinated pets are welcomed to visit. We also offer a wide variety of on-site activities and socials, so we have something to suit each individual’s needs and interests; and we invite family and friends to participate in our social activities with their loved ones.

We offer physical, occupation and speech therapy 5 to 6 days a week based on the physician’s plan of care. Each resident receives a custom recovery plan, designed to recover and maximize their independence when engaging in daily living activities.

Pathway to home

The Pathway to Home Program is designed to assist residents with a safe transition back to the community as soon as possible. This program is designed for the short stay rehabilitation patients. It provides measurable data on each patients’ progress and functional gains made during the rehabilitative stay at our facility.

Long-term Care

If your loved one is no longer able to remain at home, Glades Health Care Center provides a nurturing environment within which to help them live fully. Please feel free to consult with our business office or social worker regarding the process to start a Medicard long-term care application.
